對(duì)投標(biāo)人的資格和業(yè)績(jī)要求：

（1）資格要求

Requirements for Qualification of Bidder

A．如果****公司，投標(biāo)人需提供合法有效的企業(yè)法人營(yíng)業(yè)執(zhí)照、稅務(wù)登記證及組織機(jī)構(gòu)代碼證或證照合一的營(yíng)業(yè)執(zhí)照，投標(biāo)時(shí)需提供原件掃描件（原件備查）；

如果****公司，****公司登記注冊(cè)證明；

A．The bidder within the customs territory shall provide valid independent legal person business license, tax registration certificate, organization code certificate, or combined certificate and business license. The scanned copy of the original shall be provided during bidding (the original copy for reference);

The bidder outside the customs territory shall provide valid certificate to prove valid business registration certificate.

B. 投標(biāo)人應(yīng)為本次投標(biāo)所投產(chǎn)品的制造商或代理商；如投****集團(tuán)負(fù)責(zé)銷售投標(biāo)****公司、****公司（投標(biāo)人應(yīng)提****公司證明上述事項(xiàng)的證明文件），則視投標(biāo)人為制造商。投標(biāo)****工廠生產(chǎn)且使用相同品牌時(shí)，予以接受；若投標(biāo)人為代理商，投標(biāo)人應(yīng)得到本次投標(biāo)所投產(chǎn)品的制造商同意其為本次投標(biāo)提供該產(chǎn)品的合法正式授權(quán)書。

The bidder shall be the manufacturer or authorized agent of the bidding products.If the bidder is a wholly-owned subsidiary or holding subsidiary of the same group as the manufacturer responsible for sales of the bidding products, the bidder shall provide relevant evidence documents from the manufacturer or its superior company to prove the above matters, and in such case the bidder shall be deemed as the manufacturer. The bidding product produced by different factories of the manufacturer with the same brand shall be accepted.If the bidder is the Agent,the bidder shall obtain the manufacturer's consent to provide the product with the legal official authorization for the bid.

（2）資質(zhì)要求Qualification Requirement

投標(biāo)人必須提供制造商有效期內(nèi)的《中華人民**國(guó)特種設(shè)備生產(chǎn)許可證》，投標(biāo)產(chǎn)品在有效期內(nèi)的《特種設(shè)備型式試驗(yàn)證書》(TSX證書)。特種設(shè)備型式試驗(yàn)證書在類別、公稱壓力、公稱尺寸和溫度范圍能夠覆蓋招標(biāo)物資。（證書中制造單位名稱、地址和投標(biāo)產(chǎn)品的制造單位名稱、地址應(yīng)一致）。

The bidder must provide the manufacturer's valid "People's Republic of China Special Equipment Production License" and the valid "Special Equipment Type Test Certificate" (TSX Certificate) for the bid products. The certificate must cover the required product category, nominal pressure, nominal size, and temperature range specified in the bidding materials. Additionally, the manufacturer's name and address on the certificate must strictly match those of the bid product's manufacturer.

（3）業(yè)績(jī)要求Requirement for Bidder's reference:

2015年1月1日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人在中國(guó)境內(nèi)應(yīng)具有至少2個(gè)合同均不少于4套一體式減溫減壓器的合同業(yè)績(jī)，2個(gè)業(yè)績(jī)均至少包含1臺(tái)入口×出口尺寸不低于16寸×20寸的一體式減溫減壓器和一臺(tái)公稱壓力不低于CL600的一體式減溫減壓器。

同一集團(tuán)全球范圍內(nèi)不****集團(tuán)****公司****公司****工廠）制造的所投產(chǎn)品的業(yè)績(jī)，視為投標(biāo)人所投產(chǎn)品的制造商的業(yè)績(jī)；投標(biāo)人應(yīng)提供說(shuō)明函，說(shuō)明本次所投產(chǎn)品的制造商與滿足上述業(yè)績(jī)要求的產(chǎn)****集團(tuán)。

若投標(biāo)人為所投產(chǎn)品的代理商，則除以上業(yè)績(jī)要求外，還需具有近三年閥門的銷售業(yè)績(jī)，并提供相關(guān)業(yè)績(jī)證明文件。

From January 1, 2015, to the bid submission deadline (based on the contract signing date), the bidder must have at least two contract performance records in China, each involving no fewer than 4 sets of integrated desuperheating and pressure-reducing units. Both performance records must include:

At least one integrated desuperheating and pressure-reducing unit with an inlet × outlet size of no less than 16" × 20";

At least one integrated desuperheating and pressure-reducing unit with a nominal pressure rating of no less than CL600.

The performance of the products submitted by different manufacturers within the same group (i.e., wholly-owned subsidiaries, holding subsidiaries or manufacturing plants under the same group) is regarded as the performance of the manufacturer of the products submitted by the bidder; the bidder should provide an explanatory letter to state that the manufacturer of the products submitted in this bid and the manufacturer of the products meeting the above performance requirements belong to the same group.

If the bidder is an agent for the product being bid, in addition to the above performance requirements, they must also have sales performance of valves in the past three years and provide relevant performance certification documents.

投標(biāo)人須按規(guī)定格式提交業(yè)績(jī)表，并提交相關(guān)業(yè)績(jī)證明文件。業(yè)績(jī)證明文件包括：1）銷售合同和2）用戶簽字或蓋章的到貨驗(yàn)收材料（到貨驗(yàn)收單或調(diào)試驗(yàn)收?qǐng)?bào)告或其他可以證明合同項(xiàng)下貨物已經(jīng)到貨驗(yàn)收的有效證明材料）。投標(biāo)人所提交的業(yè)績(jī)證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁(yè)（國(guó)內(nèi)貿(mào)易合同應(yīng)有雙方蓋章，國(guó)際貿(mào)易合同應(yīng)有雙方簽字或蓋章）、制造商名稱、貨物名稱、貨物數(shù)量、入口尺寸、出口尺寸、壓力等級(jí)、閥門型式。

若業(yè)績(jī)合同為年度協(xié)議，除提供年度協(xié)議外，還應(yīng)提供相應(yīng)的已到貨訂單，訂單內(nèi)容或編號(hào)應(yīng)與年度協(xié)議相關(guān)聯(lián)。同一個(gè)年度協(xié)議下提供1個(gè)或以上的訂單及與訂單對(duì)應(yīng)的到貨驗(yàn)收材料均算為1個(gè)有效業(yè)績(jī)。

The bidder shall submit the performance record form in the prescribed format along with relevant supporting documents, which must include:sales contracts and delivery acceptance materials signed or sealed by the end-user (such as delivery acceptance forms, commissioning acceptance reports, or other valid documentation proving goods acceptance under the contract);all submitted proof documents must contain at minimum:the contract signing date, executed signature pages (with both parties' official seals for domestic contracts, or signatures/seals for international contracts), Manufacturer Name, Product Name, Goods Quantity,Inlet Size,Outlet Size, Pressure Rating and Valve Type.

If the performance contract is an annual agreement, in addition to providing the annual agreement, corresponding received orders should also be provided, and the order content or number should be associated with the annual agreement. Providing one or more orders and corresponding acceptance evidence under the same annual agreement is considered as one valid performance.

未提交業(yè)績(jī)證明文件，或通過(guò)所提供的業(yè)績(jī)證明文件無(wú)法認(rèn)定滿足上述業(yè)績(jī)要求的，均視為無(wú)效業(yè)績(jī)。

If no documentary evidence furnished, or documentary evidence furnished can not demonstrate the above performance requirements, the relevant reference will be deemed invalid.
